During my appointment’s with Taylor’d Exercise Physiology I understand the importance of providing on-going information relevant to my health for the duration of participation with Taylor’d Exercise Physiology.
I give consent for Taylor’d Exercise Physiology to record and store notes, medical information, and other pertinent information related to services received as required by ESSA, private health care fund or any other relevant body.
Furthermore as a result of signing this disclaimer in my first appointment at Taylor’d Exercise Physiology I understand, acknowledge and agree that;
(i) I am aware that the facilities and services offered by Taylor’d Exercise Physiology involve risk, included but not limited to, risk of bodily injury;
(ii) I agree that any cancellations that occur 24 hours or more before the scheduled time are able to be re-booked at no additional cost myself. The first time a cancellation is made less than 24-hour prior to the time of the booked appointment, I am aware I will be given the opportunity to re-book (with no financial penalty applied). However, any subsequent cancellations by/for myself that occur within the 24-hour period may be charged a cancelled booking fee of 75% of total consultation fee. This fee will either be payable on behalf of myself or my funding body.
(iii) I have provided above all the relevant information regarding my medical history and current health status;
(iv) I am making use of the fitness facilities and services of Taylor’d Exercise Physiology of my own free will;
(v) I assume all risks associated therewith.
