Medicare, DVA, NDIS & Workcover patients

Medicare (Chronic Disease Management Plan)

A Chronic Disease Management (CDM) plan will provide an individual with a chronic condition (>6 months) with up to 5 sessions for Exercise Physiology.

These sessions involve a referral from your General Practitioner and may include:

If your GP has provided you with a CDM plan there will a $10 out-of-pocket cost for up to 5 sessions of Exercise Physiology.


Department of Veteran’s Affairs (DVA)

If you have a DVA card (White or Gold), this will allow you access to treatment by an Exercise Physiologist for your accepted conditions. Whether this is for assistance with chronic disease, limited mobility, balance issues or decreasing strength levels, an Exercise Physiologist can help. 

A referral to an Exercise Physiologist under a DVA referral (with a D904 referral form) will give you access to fully covered (no out of pocket) sessions 2-3 times a week focussing on achieving your goals.

As of October 1st 2019, the referral system for DVA has changed; All referrals (excluding TPI) will be for 12 sessions as opposed to a 12 month continuing care referral.


NDIS

If you are eligible to receive funding under the National Disability Insurance Scheme (NDIS) then you may be able to participate in sessions with an Exercise Physiologist. 

Exercise Physiologists are trained to be able to treat and help educate individuals with many conditionsranging from neurological disorders to spinal cord injuries. 

Individuals with funding under the NDIS are able to have an allocation for ‘capacity building supports’ making them eligible for Allied Health Provider funding. The decision of where to allocate these funds can be made by the individual and/or their support coordinator. 


Workcover 

If you have been injured at work and need assistance returning to full time employment hours, Exercise Physiologists can help with the rehabilitation of your injuries and assist you with returning to your career. 

Exercise Physiologists can be very helpful with reducing the burden of your injury on your financial life and can help to take the stress out of finances by allowing you to return to work sooner.
