Medicare (CDM) 

Item Number: 10953

  • Maximum number of 5 sessions each calendar year that are either fully or partially covered with a Medicare rebate.

  • Initial Consult up to 1 hour

  • Follow Up Consult up to 30 mins

  • $10 out of pocket fee per appointment

Department of Veterans Affairs (DVA) 

Item Numbers: 

  • EP10 - Initial Consultation

  • EP11 - Follow Up Consult

  • EP33 - Group Session

Individual will be in possession of DVA white or gold card and treatment based on different cards.

Allied Health Referrals 

We encourage referrals from all Allied Health Practitioners and are committed to communicating regularly with all stakeholders in regards to patient treatment and progress.

Type 2 Diabetes

Item Numbers:

  • 81115

Type 2 Diabetes Referrals are group based sessions for groups between 2 and 12 people. 

Individuals will have access to 8 additional sessions on top of 5 sessions included in a CDM referral.

Cost:

  • Initial individual assessment - Bulk Billed (no out of pocket)

  • 8 x Group Sessions -  $40 upfront ($5/session)
